There should be a first aid centre, at every 15-20 kms on highways. These First Aid Centres should have a strategic location and have Trauma Registry to evaluate the Trauma Care.
According to the statistics available in Karnataka, more than 7000 to 8000 accidents occur every year. Nearly 10% of them are fatal. Head and brain injuries constitute almost 70 per cent of the injuries and the average time taken from the time of accident to the initiation of the treatment in cities like Bangalore is more than three hours.
In response to this alarming situation, private hospitals, especially those close to the Sate highway should form a voluntary organization called Comprehensive Trauma Consortium (CTC). CTC already exists in Bangalore and is providing organized trauma care delivery system. Leading hospitals can take a lead by forming a social and non-profit organization in lines with CTC which works under the aegis of Bangalore Neurological society.
The main objective should be to reach the accident victim at the right time (golden hour), to the right place, by right people using a right method. The need for pre-hospital care by following three principles decentralization of trauma care, participation of individuals and reorganizing the existing system for treatment cannot easily be ignored.
In the initial phase hospitals close to the state highway should be networked through wireless communication system with a dedicated frequency.
By dialing the toll free helpline number or through sms anybody who witnesses an accident can send the message to police central Control Room which has been established at Police Commissioner's office. The City can be divided into 3 or 4 zones with each zone having a representative hospital, which can handle all types of injuries including poly trauma under one roof. In addition to zonal centres, there will be several other hospitals in the zone designated as "Area hospitals" to help the first aid centres to handle minor injuries.
All the zonal hospitals can be connected through wireless system with a dedicated frequency and with a base station in every casualty department. Ambulances can be placed at the hospitals and strategic locations in the city. Every ambulance should be well equipped with a locally designed first aid kit which can be carried to the accident spot. A wireless set in the ambulance should be installed to communicate to hospital. Global Positioning System (GPS) can be adopted for effective and faster deployment of ambulances when accidents are reported. To hasten the process of pre-hospital treatment in cases of traffic jams there can be a tie-up with any private aviator for air ambulance facility. A temporary helipad can be established close to the Mysore-Bangalore State highway. Negotiations can be held with HAL for air ambulance services.
