The Western Railway (WR) on Monday said prima facie, the motorman, guard and loco inspector were collectively at fault for Sunday’s accident in which a suburban local rammed the buffer, breached part of platform and mounted on to the concourse of Churchgate station here.

Immediately after the accident, in which five persons were injured, the WR administration placed motorman Laxmi Shankar Tiwari, guard Ajay Gohil and loco inspector M G Verma under suspension pending a detailed probe.

Central Railway General Manager S K Sood, who also holds the additional charge of WR, has ordered a thorough probe into the accident to explore all possible causes and suggest remedial measures.

Mumbai Central’s Divisional Railway Manager Shailendra Kumar said detailed investigations were underway.
