<p>The Western Railway (WR) on Monday said prima facie, the motorman, guard and loco inspector were collectively at fault for Sunday’s accident in which a suburban local rammed the buffer, breached part of platform and mounted on to the concourse of Churchgate station here.</p>.<p><br />Immediately after the accident, in which five persons were injured, the WR administration placed motorman Laxmi Shankar Tiwari, guard Ajay Gohil and loco inspector M G Verma under suspension pending a detailed probe. <br /><br />Central Railway General Manager S K Sood, who also holds the additional charge of WR, has ordered a thorough probe into the accident to explore all possible causes and suggest remedial measures.<br /><br />Mumbai Central’s Divisional Railway Manager Shailendra Kumar said detailed investigations were underway. </p>
