If a person is 45 years to 59 years and has any one of the 20 comorbidities, he/she will be eligible for Covid vaccination.</p>.<p>These comorbidities were selected for priority vaccination as they enhance the risk of mortality when an individual contracts Covid-19.</p>.<p>The 20 comorbidities include nine heart conditions.</p>.<p><strong>Co-morbidities</strong><br />1. Heart failure with hospital admission in past one year<br />2. Post cardiac transplant/left ventricular assist device (LVAD)<br />3. Significant left ventricular systolic dysfunction (LVEF <40%)<br />4. Moderate or severe valvular heart disease<br />5. Congenital heart disease with severe PAH or Idiopathic PAH<br />6. Coronary artery disease with past CABG/PTCA/MI and hypertension/diabetes on treatment<br />7. Angina and hypertension/diabetes on treatment<br />8. CT/MRI documented stroke and hypertension/diabetes on treatment<br />9. Pulmonary artery hypertension and hypertension/diabetes on treatment<br />10. Diabetes (> 10 yearsORwith complications) and hypertension on treatment<br />11. Kidney/ Liver/Hematopoietic stem cell transplant: Recipient/On wait-list<br />12. End stage kidney disease on haemodialysis/CAPD<br />13. Current prolonged use of oral corticosteroids/immunosuppressant medications<br />14. Decompensated cirrhosis<br />15. Severe respiratory disease with hospitalizations in last two years/FEV1 <50%<br />16. Lymphoma/ Leukaemia/ Myeloma<br />17. Diagnosis of any solid cancer on or after 1st July 2020 or currently on any cancer therapy<br />18. Sickle cell disease/bone marrow failure/aplastic anemia/thalassemia major<br />19. Primary Immunodeficiency Diseases/HIV infection<br />20. Persons with disabilities due to intellectual disabilities/muscular dystrophy/ acid attack with involvement of respiratory system/persons with disabilities having high support needs/multiple disabilities including deaf-blindness</p>.<p><strong>Registration on Co-WIN 2.0</strong><br />1) Advance self-registration can be done through cowin.gov.in or Arogya Setu app<br />2) Beneficiary can choose Covid vaccination centre of their choice with date and time of the available schedule<br />3) Onsite registration will also be available<br />4) With one mobile number, a person can register at most four beneficiaries with nothing in common except the mobile number<br />5) The ID card number for each such beneficiary must be different<br />6) All beneficiaries, are advised to carry any one of the following photo ID documents:</p>.<p>• Aadhar Card</p>.<p>• Electoral Photo Identity Card (EPIC)</p>.<p>• The Photo ID card specified at the time of registration in case of online registration (if not Aadhar or EPIC)</p>.<p>• Certificate of co-morbidity for citizens in age group of 45 years to 59 years (signed by a registered medical practitioner)</p>.<p>• Employment certificate/ Official Identity Card – (either but with photo and date of birth) for HCWs and FLWs</p>.<p>7) Around 10,000 private hospitals empanelled under Ayushman Bharat PMJAY, more than 600 hospitals empanelled under CGHS and other private hospitals empanelled under State Govts' Health Insurance Schemes can participate as Covid Vaccination Centers (CVCs).<br />8) A list of all these private hospitals has been uploaded on the website of Ministry of Health and Family Welfare and National Health Authority</p>
